Child Sexual Abuse Survey
You are invited to participate in a survey on child sexual abuse. Results from this survey may give us information vital in our fight to protect children from sexual abuse. This survey will ask you questions about your personal experience, if any, with child sexual abuse, and also about any known abuse or abusers within your family or extended family. No identifying information will be asked. Your participation is anonymous and your responses will be added to and only reported with the responses of thousands of other participants. Completing this survey takes approximately 10 minutes.

Participating in this survey is voluntary and you may withdraw your participation at any time. A potential risk to participating in this survey is that it could cause you emotional distress by asking you to recall and disclose sexual abuse that happened to you as a child or that you are aware of within your family. A potential benefit to participating in this survey is your knowledge that you have contributed information which may be useful toward protecting other children from sexual abuse. If you feel uncomfortable answering any questions in this survey, you may withdraw from the survey at any point by clicking “Exit and Clear Survey.” If you click “Exit and Clear Survey,” your responses will not be recorded.
